1. I represent that all the information supplied in this form is current and accurate.
2. I understand that the submission of any false and/or inaccurate information may result in the revocation of my permit.
3. I represent that I have reviewed and understand the Permit Guidelines (can be found at https://preservationparks.com/natural-resources/#scientific-research) and agree to submit my current specimen collection data/results and other written research material in regard to this project by the appropriate date.
4. I understand that Preservation Parks of Delaware County may disseminate my written research information at its sole discretion without prior approval from me.
5. I represent that myself and all persons listed on the permit have signed and dated the Release and Waiver and have submitted the Release and Waiver with this application. The research permit application process/review will not begin until all release and waiver forms are received completed.
6. I understand that a failure to submit a Final Report may prevent me from obtaining any future Scientific Research Permits with Preservation Parks of Delaware County.
